INSTRUCTIONS FOR AWARDS APPLICATION
- The awards application form should be filled up and submitted online
- All entries should be supported by relevant documents
- A good passport size photo should be mailed with application
- WDC2019 may request for more documents and photos if needed
- All applications should be sent by mail only to awards@worlddentalconference2019.com
- Please attach a brief bio data and relevant documents ( certificates of achievements, experience etc) to improve your chances for the award.
